Saltzer Medical Group now provides foreign travel vaccines. Saltzer’s Foreign Travel Vaccine Department provides comprehensive immunizations and health education services for area residents who are planning international travel.

When planning an overseas excursion please complete this form, and return to Saltzer.

The Immunization Coordinator will schedule an appointment for your travel immunizations.

World Health organization cards will be provided to those receiving Yellow Fever vaccinations.

Previous immunization history
A detailed travel itinerary that includes your travel destinations and the dates you will be visiting.
